Daily and Weekly Log

The purpose of the Logs:

• To keep a daily or weekly record of the clients and therapeutic interventions performed.
• To assist the student to track the skills they are developing relating them to the clients’ diagnoses being treated.
• Daily or weekly log assists in tracking skills being practiced and assist with accurately recording them on the Practical Skills Checklist.

Student will submit only a signed OTA&PTA Clinical Documentation & Caseload Management Form to the required ECentennial Drop box at the end of placement to verify documentation. The documentation completed on placement is NOT to be submitted to Centennial College.

If required the following document includes the student and Clinical Supervisor instructions, examples, and blank template to be used as a worksheet for practice.
