What is a One Page Profile?

A one-page profile is a personalized introduction to a person.  It is an ‘at-a-glance’ way of knowing what people admire about them, what matters most to them and how to best support them.

This course will discuss one-page profiles for a DSW and one-page profiles for those you will be supporting.

The content is the same for both types of one-page profiles, but the focus will be slightly different.

Below is an example template for a one page profile. Click on each purple (+) to learn more information about what goes on in each section and to access explainer videos for each section. Once you have reviewed all of the content on this page, you will have enough information to begin to develop a one-page profile.


Below are example templates (Microsoft Word, Microsoft PowerPoint & Canva) for a one-page profile. As you learn the other tools in this interactive book, you will discover more information that can be added to each section.

You can select and access the one-page profile templates down below to create your own. A one-page profile must represent the person. This is your chance to get creative. Can you change the background images or colour? Should you choose specific fonts or font colours? What photos can you add to their profile to really make it stand out as their own?

Person Centered Plan Task

  1. Complete your own 1 Page Profile and be prepared to quality-check it in next week’s class.
  2. Meet with your focus person and complete a one-page profile with them. Each week, you will continue to add and adjust their one-page profile based on the other tools you will complete with them.
  3. Ask the focus person the following questions: a) How much of what is important to you is actually happening in your life? b) Are you being supported in ways that work for you?
  4. Add any possible Notes for Actions. These may become goals in the Person Centered Plan.
  5. Review Friday Knights One Page Profile Example below.
