Student Evaluation of Fieldwork Experiences

The purpose of the Student Evaluation of Fieldwork Experience form is to

• Facilitate thoughtful reflection on all aspects of placement, including attainment of goals.
• Assist in critical thought around future goals and placement expectations.
• Provide feedback allowing the supervisor, facility, and college to improve students’ learning experiences.
