Document Brief Description
Student Placement Profile
  • Students are to complete their student profile and send it with their introductory email.
  • Provides a snapshot of learners experiences up to this point, including any previous placements or employment.
  • Allows student to provide information on how they best learn and receive feedback.
  • Can assist in planning of placement opportunities, skill development and goal setting.
Practical Skills Reflective Assignment
  • Students are to set 3 SMART goals in collaboration with their Clinical Supervisor after first week of placement.
  • Complete Skills Table and goal attainment reflection at mid-term and final.
Clinical Structured Notes
  • Students will complete three (3) written notes in e.g. SOP, DARP, PCARE, or any structured format used by your facility, to be corrected.
  •  Students will continue to practice writing progress reports and when able to produce three (3) error free note, please complete the OTA&PTA Clinical Documentation & Caseload Management Form. 
  • Student will submit 1 signed OTA&PTA Clinical Documentation & Caseload Management Form to Program Coordinator/ Course Instructor via the course dropbox at the end of placement to verify documentation.
Daily or Weekly Log Sheet
  • To keep a daily or weekly record of the clients and therapeutic interventions performed to assist the student to track the skills they are developing.
  • Student will submit 1 signed OTA&PTA Clinical Documentation & Caseload Management Form to Program Coordinator/ Course Instructor via the course dropbox at the end of placement to verify caseload management.
Canadian OTA &/or PTA Student Fieldwork Evaluation Form
  • To be completed at midterm and upon completion of placement. Both are reviewed with the student.
  •  Provide student with a copy for their portfolio. Semester 2: only final evaluation required for submission.
Student Evaluation of Fieldwork Experience
  • Student is to complete by end of placement and share information with the Clinical Supervisor and the Program Coordinator (via drop box).
  • Providing feedback allows the supervisor, facility, and college to improve students’ learning experiences.

*Please Note: Structured notes and Log sheets are NOT to be submitted. In their place, the OTA&PTA Clinical Documentation & Caseload Management From is to be submitted for verification.*

Upon completion of the placement, the Canadian OTA and/or PTA evaluation form can be completed digitally, or if in paper format scanned, with signatures. Completed forms are submitted electronically from the facility’s email address and have a notation that it has been reviewed with the student. Email to kkoseck@centennialcollege.ca

Students will be expected to have a copy of all evaluation forms for their portfolio . Students will need to upload signed/final copy of the Canadian OTA and/or PTA evaluation form to course dropbox.  Any specific questions or concerns regarding the student’s progress or evaluation should be directed to the Faculty Advisor assigned to the student.
