Prior to Placement

☐Include onsite supervisor, offsite clinical instructor and faculty member on introduction email to site

☐Follow-up with onsite supervisor if there are any learning opportunities to prepare for the placement

☐Complete learning modules on-line in course shell under Emerging Role Placements

☐Review fieldwork placement manual

☐Review student email and suggest any learning opportunities specific to the site that may help student prepare

☐Meet with offsite supervisor if needed to arrange process to obtain client consent, review goals for placement and discuss student access to office space and technology

☐If possible, obtain client consent to consultation

First Week of Placement

☐Participate in any orientation activities and check about working spaces.

☐Establish SMART goals as part of Practical Skills Reflective Assignment

☐Establish communication strategy for onsite supervisor and offsite clinical instructor

☐Complete daily clinical notes and daily/weekly log to submit to offsite clinical instructor

☐Co-develop learning objectives (SMART goals) with offsite clinical instructor and student

☐Establish method for tracking placement hours (e.g. student coordinator at site or sign in process at facility)

☐At end of each week, after student completes hours, onsite supervisor signs form to validate hours

☐Obtain consent from clients for consultation services

☐Assign OTA duties

☐Co-develop learning objectives (SMART goals) with onsite supervisor and student

☐Review daily clinical notes (e.g. SOAP notes) and sign off.

☐Sign off on one daily/weekly log for student to submit to college

Second Week of Placement

☐Finalize SMART goals

☐Continue to complete daily clinical notes and daily/weekly log to submit to offsite clinical instructor to review

☐Finalize learning objectives (SMART goals) with offsite clinical instructor

☐Assign day-to-day duties to student

☐Continue to assign OTA duties to student

☐Finalize learning objectives (SMART goals) with onsite supervisor

☐Continue to review daily clinical notes and sign off

Mid-Placement

☐Complete self-evaluation and discuss with onsite supervisor and offsite clinical instructor

 

☐Complete electronic version of student evaluation and review with student and offsite supervisor mid placement ☐Complete electronic version of student evaluation and review with student and offsite supervisor mid placement
Last Week of Placement

☐Complete self-evaluation and discuss with onsite supervisor and offsite clinical instructor

☐Provide placement supervisor with all clinical documentation that has been signed off by offsite clinical instructor

 

☐Complete electronic version of student evaluation and review with student and offsite supervisor

☐Collect clinical documentation and secure/store on site

☐Complete electronic version of student evaluation and review with student and onsite supervisor

☐Sign off on any clinical documentation
