10 Consent Form for Student Contributor

Agreement: Contribution to Open Educational Resource

I,_______________________________________, agree to participate in the creation of an open access book on Shakespeare and adaptation, in collaboration with [name of OER creator(s) – in our case this was the instructor plus the student partner].
I agree to provide [university name] a non-exclusive copyright license of the final project I created in [course number/title], for the purpose of creating this open educational resource.

I understand that the book and my contribution to it will be subject to Creative Commons Licenses. I have read the Guide to Creative Commons Licenses https://creativecommons.org/about/cclicenses/ and understand the differences between a CC BY-NC license and a CC BY-NC-ND license. I understand that inclusion of my work is conditional upon my willingness to license my contribution under one of the following licences (initial to the left of your choice below):

CC BY-NC
This license allows re-users to distribute, remix, adapt, and build upon the material in any medium or format for non-commercial purposes only, and only so long as attribution is given to the creator.
CC BY-NC-ND
This license allows re-users to copy and distribute the material in any medium or format in un-adapted form only, for non-commercial purposes only, and only so long as attribution is given to the creator.

I understand that I also have the option to choose how I want my contribution to be attributed: to my real name, to a pseudonym, or to “anonymous” (initial to the left of your choice below and fill in the name you wish to appear in the book, if relevant)

I understand that inclusion of my work in the final text in this open educational resource is conditional upon my willingness to edit my text as initially specified by [OER authors’ names] on or before [date]. I understand that I will be given the opportunity to review a draft of my contribution and make final changes to it prior to the book’s publication. Additionally, I understand that I have the right to request that my name and/or work be removed from the text, or change the license on my contribution, at any stage prior to publication.
I understand that due to practical considerations relating to the open nature of this resource, I will not be able to withdraw my consent granting [university name] non-exclusive copyright to include my contribution in the book after it is published.
