1.     Uses electric wheelchair to move around

3.     Unsteady Gait

Diagnosis

Impaired mobility related to unsteady gait as evidenced by use of electric chair

 

Goal

In a week resident will use different methods to improve independence within their limits.

Patient will improve physical mobility by consistent monitoring of interventions to ensure the best results and will verbalize an increase in strength within a year.

 

Interventions:

 

1. Encourage patient mobility and to exercise according to a plan tailored to fit their needs and is within patient limits

 

2. Assess client on daily basis

 

3. Encourage use of assistive devices and independence while using them

 

Rationale:

 

1. Helps with pain and helps increase and improve mobility

 

2. to identify any additional risk factors that may result in decreased physical mobility

 

3. keeps patient safe, does not act as a restraint and assists in improving patients’ mobility

Always using assistive devices, without the help of healthcare worker. Shows independence by going on walks using electric wheelchair and knitting regularly.
