8 Academic and Non-Academic Placement Requirements

Non-Academic Pre-requisites (NAP) and Academic Pre-requisites

Description: To attend work placement activities several pre-requisites are needed to first determine eligibility and then to support the requirements outlined by healthcare settings – work placement hosts.

Documents and Forms 

  • Acknowledgment Form-start of participation
  • Freedom of Information Form-start of participation
  • Resume and cover letter-start of participation
  • Immunization Form – after securing hospital placement
  • WSIB Student Declaration Form-student signed form after securing placement)
  • WSIB Work Placement Host Declaration Form – work placement host to sign after securing work placement)
  • Placement Hours Tracking Form (completed during placement, submitted at end of placement)

Placement Requirements Non-Academic Pre-requisites (NAP)

  • Resume and cover letter to apply most placement require this – prepare at start of placement
  • Follow application process for placement opportunities
  • Follow interview protocols as established by work placement hosts
  • Orientation with work placement host (if applicable)
  • Police Check/Criminal and Judicial Matters check (if applicable) see Police Screening Chapter
  • Hospital Requirement (some community placements require this as well)
    • Immunizations* (if applicable) Hospital Requirement see Hospital Immunization Checklist – below
    • Measles, Mumps, Rubella
    • Varicella or titre (if status of immunization is unknown)
    • Hepatitis B
    • Td – tetanus, diphtheria
    • Flu vaccine
    • Covid vaccination (discontinued April 2024)
    • TB testing (two step within the last 12 months)
    • Mask Fitting Hospital Requirement (if applicable)
    • Police Check – Criminal and Judicial Matters Check

 

Important Notes*

*Proof of immunity to Varicella, Measles, Mumps, Rubella (2 doses), Hepatitis B, and Tetanus Diphtheria, Pertussis as well as documentation of a 2-step TB test is required. **It is important to note these are standard requirements for hospital employment opportunities. If you are seeking employment at a hospital after graduation completing these steps will remain valid for 12 months.

Learners who do not have a healthcare provider or who require immunization, or have questions surrounding immunization status various walk-in clinics can assist you. Grove Street Walk-in Clinic is familiar with immunization requirements they are located close to the campus here is their website for location and hours: Grove Street Walk-in Clinic.  There may be a cost upfront and billing can be submitted to Guard.Me for international learners.

Placement Requirements Academic Pre-requisites (AP)

  • Keyboarding 45 wpm
  • Successful completion of all semesters 1, 2, and 3 course and registered full-time in semester 4. Meaning program will be concluded by the end of semester 4 (transcript)
  • Privacy Certificate (OFAD 2025 mandatory completion of this certificate)
  • Worker Health & Safety Certificate (online training module in 4 steps completed in Professional Development in year 1)
