23 Workplace Evidence Supervisor Verification

Some students may identify workplace projects that demonstrate academic competency requirements. If so, you will initiate a form in PebblePad that will require your workplace supervisor verification before submission to the faculty.

Students are required to first ensure they have your permission to submit specific workplace projects by you.

The Student Guide video will assist you in this process.

Below is the student agreement:

By electing to submit a sample of your work for academic assessment consideration, please confirm that you have obtained approval from your supervisor to share the information provided herein. The workplace materials submitted for faculty review will remain confidential and will not be disclosed to unauthorized parties. I understand and acknowledge that my workplace supervisor will review the contents of the workplace evidence prior to their release to the faculty at York University for feedback and verification.

Below is a copy of the Employer Service Agreement for the Digital Technologies program with reference to privacy and confidentiality.

The parties acknowledge that because of this agreement they may exchange confidential, proprietary or sensitive documents and information, including personally identifiable information. The parties agree to treat all information exchanged as sensitive and confidential, whether or not marked as such, and shall take reasonable precautions to ensure it is not disclosed to any third party and to limit internal disclosure to those employees who require access. The obligations of this section will bind a party for so long as that party retains any such documents and/or information.

Notwithstanding any other term of this Agreement, the parties agree that the University may have independent obligations under the Freedom of Information and Protection of Privacy Act in effect from time to time (“FIPPA”) or other legislation and that nothing in this Agreement will be construed to require either party to breach FIPPA’s terms. The parties will provide reasonable cooperation to each other to permit them to meet their privacy and confidentiality legal obligations if and as they arise.
