7.6 Incident Reporting and Injury Reporting Process and Forms

If you are attending your Clinical Professional Practice Placement and you are injured, please advise your clinical instructor immediately. Your instructor will notify the agency, the PSW placement coordinator and the PSW program coordinator. Your instructor will connect with you and the agency to complete the appropriate Incident Form(s).

Injuries and potential hazards can be reported on the Environment, Health, Safety & Emergency Services page of the fanshaweca.ca SharePoint website.
