Person Centered Plan Task
Create a Good Day / Bad Day Chart with your focus person.
Once created:
- Update their One Page Profile with important information discovered with this tool.
- Ask these questions: a) Look first at all of what makes a good day for you. Is there anything you or others could do to have more of what is on your list or for it to happen more often? b) Now look at your bad day list. Is there anything that you or others could do to prevent some of those things from happening or make them happen less often? c) Make notes about what you want to do or what others could help you with to have more good days and fewer bad days.
- Record notes for actions. These will become possible goals for the person in their Person-Centered Plan.
