Appendix D: Teaching Observation Feedback Form (Grid)

Downloadable Version

This grid includes prompts to help the observer capture their observations and feedback during the teaching observation. Consider discussing the use of this feedback grid with the instructor ahead of the in-class observation and use the goals identified by the instructor to make modifications accordingly. The feedback grid should be completed by the observer. It is adapted from the Program for Faculty Development, Peer Observation of Teaching Toolkit, Faculty of Health Sciences, McMaster (2016).
