Appendix C: Teaching Observation Feedback Form (Structured)

Downloadable Version

This form includes broad categories to help the observer structure their observations and feedback during the classroom visit. Although the form is structured by themes, it is not meant to be a required or prescriptive list of “best” teaching practices; rather, this form outlines various areas of teaching practice that may be relevant across a wide array of contexts. Consider discussing the use of this feedback form with the instructor ahead of the in-class observation, using the goals identified by the instructor to modify it accordingly. This feedback form should be completed by the observer.

A. Classroom Management

How effectively is the classroom managed? For example, does the instructor:

  • Start class on time and welcome students?
  • Clearly communicate how to access resources, platforms, and technologies required for class?
  • Ensure their classroom is a positive, respectful, and safe learning environment?
  • Integrate appropriate breaks, depending on class length?
  • Use class time effectively?
  • End class on time?

B. Lesson Structure and Organization

How effectively is the lesson structured and organized? For example:

  • How clear are the intended learning outcomes?
  • How aligned are the lesson’s activities and assessments with the lesson’s intended learning outcomes?
  • How effectively does the instructor connect the lesson to previous or upcoming course content?
  • Does the lesson flow in a logical way?
  • How is the lesson pacing?

C. Teaching Strategies and Materials

What teaching strategies and materials does the instructor use, and how do they support student learning? For example, does the instructor:

  • Use a variety of instructional strategies effectively (e.g. lecture segments, questions, discussions, problem solving, applying concepts, providing feedback, quizzing)?
  • Use instructional materials/resources (e.g. PowerPoint slides, whiteboards, videos, readings, microphone, Teams, Zoom, etc.) that support student learning?
  • Clarify content (e.g. defining concepts, repeating or elaborating complex information, using examples) when it appears to be necessary?
  • Assess student learning and provide helpful feedback throughout the lesson?

D. Learner Engagement and Activities

How do students engage with the class content? For example:

  • What learning activities are being used in class (e.g., note taking, discussing, problem solving, polling, working in pairs or groups, other forms of active learning)?
  • Does the instructor communicate instructions and facilitate the learning activities effectively?
  • Does the instructor provide enough time for students to complete the learning activities?
  • How are students responding to the instructor and the learning activities (e.g. they appear engaged, they look bored, they look confused)?
  • Does the instructor recognize and intervene when students are disengaged, confused, or not following the lesson?
  • How effectively does the instructor invite and respond to student questions?
  • Does the instructor provide meaningful oppourtunities for quiet processing?

E. Learning Environment: Accessibility, Inclusion, and Well-Being

What is the classroom climate like? How has the instructor created or contributed to this climate? For example:

  • How do the instructor’s behaviours impact the learning experience (e.g., enthusiasm; positive reinforcement; gestures; clarity, pace, tone, and emphasis when speaking; demeanour; active listening, addressing all students)?
  • How does the instructor welcome student participation?
  • How does the instructor support student well-being (e.g., being approachable, referring to students by name, offering breaks)?
  • Are the lesson materials accessible (e.g. alternative formats are provided, images have alternative text, videos are closed captioned)?
  • Does the instructor use inclusive language (e.g. students’ stated pronouns)?
  • Does the instructor alert students to sensitive subject matter and/or manage classroom conflict effectively?

F. Student Feedback:

If collecting student feedback, please make note of that here.

F. Observer Reflections:

What did you, the observer, learn by watching your colleague teach? For example:

  • What is one thing you feel inspired to learn more about or to integrate into your teaching practice following the observation?
