School and Program Policies

Centennial College policies apply to staff, faculty and students.

The following additional guidelines are important for students in the program.

HEALTH AND WELLNESS PLACEMENT
While in a clinical setting or a professional placement, students are representative of Centennial College
and must abide by the professional expectations listed in the student code of conduct.
Students must complete placement requirements prior to taking placement courses.
Faculty can not authorize the clearance status for students. Students must contact Placement
Requirements via email.
Students must dress appropriately as instructed by their faculty and/or site supervisor.
When directed, students must wear the designated name tags to identify themselves.
Students must respectfully follow the rules of the host organizations.
Sites may require students to come for an interview or for an orientation. Students are expected to
attend these as per direction of the site coordinator or faculty advisor.
Attendance, punctuality and professionalism are an expectation. Students failing to attend placements,
being late, leaving early or being disengaged or disrespectful at the placement, will be withdrawn from
the site.
In case of an emergency or illness, students must notify the site supervisor and the faculty advisor
as quickly as possible. The faculty will work with the student to create a work plan for missed hours.
In case of unforeseen circumstances, where the student may be late in arriving, the student must
immediately contact their site supervisor notifying them of the delay and the estimated time of
arrival. The student must also contact the faculty advisor to discuss the make up time for missed
placement hours.
It is the student’s responsibility to communicate any challenges they encounter at the placement sites to
their faculty advisor in a timely manner and seek support for mediation.

NON-COMPLIANCE
Students failing to demonstrate professionalism at the external site will be immediately removed from the
host organization. The following remedial actions and possible outcomes will occur.
Student will meet with the faculty and the associate dean to reflect on the behaviours that resulted in the
withdrawal.
Student will write a reflective paper summarizing the situation and their role in it, create concrete goals
for improvement and success, and develop an action plan for implementation.
Possible Outcomes
Outcome 1: If the reflection is deemed satisfactory and the host site is willing to accept the student
again, the student will return to the placement and complete the missing hours. If unacceptable
behaviour persists, student will be withdrawn and will fail the course.
Outcome 2: If the reflection is deemed satisfactory but the host site is unwilling to accept the student,
the program may attempt to find a new site but there is no guarantee. If an alternate site is found, the
student may be expected to start over (previously completed hours may not count). If unacceptable
behaviour persists, student will be withdrawn and will fail the course. If an alternate site is not found,
student will fail the course and will have to repeat.
Outcome 3: If the reflection is deemed unsatisfactory, the student will fail the placement course and will
have to repeat.

Police Record Check Requirement

A “Police Records Check with Vulnerable Sector” screening needs to be completed prior to placement/clinical and then repeated annually. All police record checks must be done within the region where you live at a local police department. Vulnerable person means a person who, because of their age, a disability or other circumstance, whether temporary or permanent, are (a) in a position of dependence on others or (b) are otherwise at greater risk than the general population of being harmed by a person in a position of authority or trust relative to them. Students who have a ‘positive’ police record check, may be denied placement by the agency and will therefore, not be able to complete the fieldwork placement. Successful completion of all clinical placements is a requirement for promotion and graduation.

 

Immunization Review Form

Ontario legislation specifies certain surveillance requirements for those entering into a practice setting. In order to comply with the Public Hospitals Act, other legislation and to meet the placement settings requirements, an Immunization Review form must be completed in its entirety and submitted, before the commencement of practical courses. Students who do not have a family doctor can have this form completed at a “Walk-In” Clinic.
