All documentation needs to be valid for the entire time the student is in placement.

1) POLICE RECORD CHECK REQUIREMENT

Specific instructions for arranging a Police Records Check with Vulnerable Sector Screening: This needs to be completed prior to placement/clinical. Each program will provide students with a deadline date.

The following information is provided for students who have received and confirmed an offer to a particular program and who have been instructed to arrange a Police Records Check with Vulnerable Sector Screening (PRCVSS) as required by their program.

In compliance with requests from our clinical placement partners, all Health Studies students who have a clinical placement component in their program are required to have their PRCVSS completed on a yearly basis.  All PRCVSS must be done within the region where you live at a local police departments.

In compliance with requests from our placement (practical/clinical) partners, students are required to have completed an annual criminal reference/police record check. These checks are done to protect clientele who are considered “vulnerable persons*”.

*Vulnerable person means a person who, because of their age, a disability or other circumstance, whether temporary or permanent, are (a) in a position of dependence on others or (b) are otherwise at greater risk than the general population of being harmed by a person in a position of authority or trust relative to them

2) Standard First Aid

All students are required to obtain a current certificate of completion for an approved SFA course. Most SFA courses are included with CPR courses.

3) Cardiopulmonary Resuscitation (CPR)

All students are required to obtain CPR Health Care Provider (HCP) certification on a yearly basis.

4) Respiratory Mask Fit Test

Mask fit testing will be done at Centennial College throughout the school year. Mask fit testing : will be performed by trained School staff in our Health Studies Lab- Room 258. This allows us to better manage the scheduling for mask fit testing and provide you with a considerably higher level of service.  Taking into account all of the placements for all of our programs and each program and section class schedule, we have developed a mask fitting schedule which will ensure that you have the opportunity to have this done prior t your first placement.  All students who require mask fitting will have an appointment time assigned to them, normally during the semester prior to their first placement. Your appointment time will be available through your program coordinator. Please arrive 10 minutes earlier to fill out a respirator health questionnaire.

If you miss your mask fit testing appointment: due to the large number of students requiring mask fit ting, we are obliged to centrally schedule your mask fit appointment. All students are expected to organize their schedules and activities to respect their assigned appointment time.  If you miss your appointment, you will be charged a $30 fee to rebook your appointment.  Only for documented medical or other critical reasons will this fee be waived, and only at the discretion of the Program Coordinator of your program.

5) Immunization Review Form

Ontario legislation specifies certain surveillance requirements for those entering into a practice setting. In order to comply with the Public Hospitals Act, other legislation and to meet the placement settings requirements, an Immunization Review form must be completed in its entirety and make an appointment with PARAMED, before the commencement of practical courses.

Students who do not have a family doctor can have this form completed at a “Walk-In” Clinic.

This process is necessary to ensure that those working within health care facilities are free from or are protected against communicable & infectious diseases. The completion of this form is not optional as it is required information to confirm your placement.

For safety and liability reasons, students will not be placed in clinical and field practice agencies until the above is complete. This will jeopardize the successful completion of the course & the attainment of the health sciences diploma/ certificate.

This form is to be filled out by Health care provider and can take up to 4 to 6 weeks to complete.

Yearly TB surveillance

All Health Studies students are tested for TB

Previous test negative – 1 T»B (Mantoux) test is required

Previous test positive – physicians note stating no symptoms of TB

Required immunization as set out in guidelines prepared by Ministry of Health and as detailed in the Pre-placement Health Form

Completion of a `Two Step`Mantoux test, a chest x-ray may be required depending on the results of the Mantoux test. The results submitted must be within one year of program registration date.

Polio Vaccination

Hepatitis B Vaccination

6) Influenza Immunization

Influenza Immunization is mandatory in the Long Term care settings with some hospitals also requiring proof. In the event of an outbreak at your placement, any student without the vaccination may be denied access to the facility, thereby jeopardizing successful completion of practice. Centennial College will be having a flu immunization clinic in the Fall- date to be announced.

The status of clinical/field placements and co-op work terms will be subject to public health directives and the reopening plan. Students may complete their placement requirements at sites that are permitted to open, either onsite, following appropriate health and safety protocols, or remotely. Students will be asked to read and sign a Student Informed Consent form in order to ensure that they understand their rights and the potential health and safety risks. Similarly, employers and/or placement hosts will be required to complete an Employer/Placement Health and Safety Site Checklist.

If workplaces are not permitted to open, placements and work terms may be completed remotely where feasible. In programs where placements or unpaid internships can not be facilitated through alternative/distance models, students will be allowed to complete these components at a later date.
