8 Tracking Placement Hours and Placement Forms

How do I track my placement hours?

Students are required to submit a copy of your shift schedule for Professional Practice 2. To determine this, students will need to sit down with their Preceptor to review their schedule and record your planned shifts on the Schedule Verification Form-Shift Outline Form- this form is due within the first week of placement. Students are then required to track all placement hours and shifts using the Schedule Verification Form. This form needs to be submitted weekly (Sundays). Ensure that you bring this form daily with you, as your preceptor needs to sign off daily on this form verifying that you were present for the hours and shift.

Your preceptor likely works 40 hours a week but students can work between 24 to 40 hours per week. Work with your preceptor and select the days that you will be working over the duration of placement that will enable you to complete all of your hours (168). Complete as much of the schedule as possible based on the schedule your preceptor has available. If your Preceptor doesn’t know all shifts for the whole duration of the placement schedule, record what is available and then update Canvas with an updated form once you have the information.

The Mohawk College Clinical Supervisor needs to know when all students are attending clinical placement at all times. If there are any changes in the schedule, you must update the schedule and upload to MyCanvas. Send an email to your Clinical Supervisor (CS) to inform the CS that you have uploaded a new schedule.

Do training and orientation hours count?

YES! Please ensure to record your orientation and training hours on your Schedule Verification Form as those hours will count towards your 168 hours.


What forms do I need to submit? When do I need to submit them by? Who completes the form?

Please review information within the Professional Practice 2 MyCanvas course site under Assignments. Review items listed there and make note of due dates. Connect with your clinical supervisor if you require any clarification.

Items that need to be submitted are below by the student:

  • Weekly Schedule Verification Forms need to be submitted via MyCanvas
  • Mid-term Evaluation (due week 3) submitted via MyCanvas
  • Final Evaluation (due week 7) submitted via MyCanvas
  • Final Schedule Verification Form (due week 7) submitted via MyCanvas

How do I submit forms/documents?

Schedule Verification Forms, Mid-term and Final Evaluations are to be submitted into the assigned Dropbox within your Professional Practice 2 course on MyCanvas. Check under the Assignments section. Keep a copy for your own records. If you have difficulty with submitting the forms, please contact the course coordinator or Clinical Supervisor.

Please note: You are responsible for submitting all forms/documents. Failure to submit them by the outlined deadlines on your course site may result in a suspension of your placement. If too many placement days are missed, placement will be terminated, and a failure will be assigned in the course.

Your Preceptor will need to complete the Mid-term and Final Evaluation. Please give your preceptor a copy of each at the beginning of your placement so they understand how you will be assessed. Please also ensure to give them ample time to complete the form during week 3 and week 7. Don’t wait till the last minute as due dates need to be followed.

All forms need to be signed by the student and the preceptor.
