Please note: There is a separate Accommodation Documentation Form specific to clinical placement. Student(s) requiring clinical accommodations should visit the Accessible Learning Services How to Register webpage to access more information on the documentation that will need completion and submission.

There should be no utilization of informal accommodations and the above-outlined process should be followed for all clinical accommodation requests.
