Field Placement Request Form

Students must complete the Request Form in order to receive a placement.  When completing the Request Form, students provide their address and type of placement they are interested in completing for each Field Placement.

When the Field Placement Request Form is available, students will receive the link to the webform through their Mohawk College email.  The dates the form is available for completion are also emailed to students.
