Employer Health & Safety Form
The online Employer Health & Safety Form is a mandatory requirement that the Field Placement site completes in collaboration with the student. The Employer Health & Safety Form must be completed by the site during the student’s orientation / interview or on Day 1 of Field Placement. The link to the form can be found in the mentor letter or in the confirmation of placement email sent to the site.
Where the form is not completed by the end of Day 1 of Field Placement, the student’s placement will be put on a temporary stoppage until the form is completed. The Field Placement Specialist will initiate the stoppage and confirm when the student can return to placement. Any missed time will need to be made up.
