Weekly Placement Experience Form

The purpose of this form is not only to track completed field placement hours (which are required to be signed by a field mentor), but it allows the student to reflect on the activities they completed for the past week, their accomplishments and goals for the following week. Students should devise a mutually agreeable time to meet with their Field Mentor on a weekly basis to review this form, obtain feedback and discuss next steps. Students are responsible for submitting this form on a weekly basis to Mohawk College.
