“Prep and Plan”

 

The Pre-Clinical “Prep and Plan” must be completed and submitted to your clinical faculty at the beginning of each clinical shift.  This information will ensure that you are adequately prepared for your patient assignment.

 

Medical-Surgical Diagnoses: 

hypertension, also known High blood pressure, occurs when blood pressure is higher than normal, symptoms can include fatigue, light-headedness, headaches or even____ shortness of breath (RWJBarnabas Health, n.d.).______________________________

Prediabetes is when you have higher blood sugar levels than normal, but not high____ enough to be considered type 2 diabetes. Without lifestyle changes, adults or children who are prediabetics are at high risk of developing type 2 diabetes. Symptoms include_ fatigue, Numbness or tingling in the feet or hands, Frequent infections, Slow-healing___ sores, Increased hunger or thirst, Frequent urination and Unintended weight loss  (Mayo Foundation for Medical Education and Research, 2022). _________________________

 

 

Anticipated assessment data for this patient:

List 2 priority “NANDA” Nursing Diagnoses based on the above assessment data:

 

  1. Unstable blood glucose level related to smoking as evidenced by medical diagnosis of prediabetes_______________________________________

 

  1. Risk for decreased cardiac tissue perfusion related to history of hypertension.

 

Identify specific nursing interventions to support these priority nursing diagnoses: (action plan)

Based on your prep & plan what will you pay special attention to: (What do you see as the patient’s priority issue?)

 

The patient’s priority issue is likely the risk for decreased cardiac tissue perfusion. I will monitor blood circulation by assessing skin condition assess vital signs especially such as heart rate, blood pressure and respiratory rate. I will also ensure the physician is contacted to see if any medication is needed to administer to manage patients’ health. Lastly, I will observe the patients daily activities and encourage a healthy lifestyle.

     

 

 

 

What teaching do you think your patient will require?

 

I will provide education to the patient regarding the effects of high blood sugar and pressure and how beneficial a healthy lifestyle can be, these will include things like the importance of exercise and a balanced diet.
