19 Day 4 Post Operative Visit
You are returning as the community nurse on day 4 to visit Janet. The patient identifiers, environmental scan and hand hygiene have been performed. Janet is concerned about the colour of her JP drainage and the redness around the incision site. Janet’s vital signs are TPR, BP, SpO2:, 37.7, 96, 18, 132/74, SpO2 97% on room air (RA). As the nurse, you assess the incision and the drainage from the JP drain.
Janet’s wound shows signs and symptoms of infection. Her temperature is 37.7, her heart rate is 96, the incision site is red and warm to touch, the JP exudate is seropurulent and Janet is feeling unwell. Janet states she has taken Tylenol, 2 hours prior to your arrival.
