AMHW Field Placement – Required Forms & Deadlines 📌

During Field Placement, students are required to complete specific forms at key stages. Each form has a clear deadline to ensure compliance and readiness for placement activities.

Form / Assignment Description Appendix Deadline Notes / Submission Instructions
Area of Interest Form Indicates your field placement preferences to help the Field Placement Coordinator match you with a suitable agency. Students list types of work and areas of interest, rather than specific agencies.
Examples: harm reduction programs, working with seniors, supporting community members experiencing homelessness, treatment centers, or other relevant sectors.
Before Field Placement (exact deadline provided via email) Complete online via the Field Placement Profile (Google Doc). Include: geographic area, accommodation or accessibility needs (including registration with CALCS), interests in specific populations (up to 3), personal details (name, address, student number, pronouns, preferred placement location), and any other relevant information.
⚠️ Failure to submit by the deadline may result in being placed on an Independent Search.
Vulnerable Sector Check (VSC) Ensures the student is cleared to work with vulnerable populations and meets agency and ministry safety requirements. Before Field Placement Submit directly to your Agency Field Supervisor. Must be completed prior to attending placement. Include valid government-issued ID, proof of address from the last 5 years, and any agency request letter. Fingerprints may be required for some students. Contact Field Placement Coordinator if any issues arise with your record.
Medical and Immunization Requirements for Field Placement Some agencies (especially in healthcare, residential, or community settings) may require medical clearance or proof of immunizations before placement begins.
Common requirements may include: TB testing, MMR, Tdap, Varicella, Hepatitis B, Influenza, COVID-19, or a medical clearance form from a healthcare provider.
Before Field Placement (if required by agency) Confirm with your agency whether medical or immunization documentation is required. Keep copies of all records for your files.
⚠️ Students are responsible for arranging tests/vaccinations and covering any associated costs (some may be covered by OHIP or student health plans).
Field Placement Contract Formal agreement between student, agency, and college outlining responsibilities. Appendix I End of Week 1 of Field Placement Submit to Field Placement Coordinator via email or in person during drop-in hours (Wed–Fri, 10:00 AM–2:00 PM, Room C206).
Student Declaration of Understanding – WSIB or Private Insurance Coverage Confirms student understanding of insurance coverage and responsibilities. Appendix J End of Week 1 of Field Placement Submit to Field Placement Coordinator via email or in person during drop-in hours (Wed–Fri, 10:00 AM–2:00 PM, Room C206).
Letter to Placement Employers Regarding WSIB Notifies placement employer of WSIB coverage requirements. Appendix K End of Week 1 of Field Placement Submit to Field Placement Coordinator via email or in person during drop-in hours (Wed–Fri, 10:00 AM–2:00 PM, Room C206).

⚠️ Tip: Stay on Top of Required Forms

  • Organize: Keep all forms in a dedicated folder—digital or physical—to bring with you on your first day.

  • Get Signatures Early: Have your Agency Field Supervisor (AFS) and/or College Field Supervisor (CFS) review and sign all forms on your first day to ensure completion.

  • Avoid Missing Deadlines: Forms must be signed and submitted by the required deadlines. Failure to do so may prevent you from continuing your placement and could result in having to drop the course.

Staying organized and completing your forms promptly ensures a smooth start to your placement and keeps you in good standing with the program.
