Appendix L – Meeting Preparation, Professionalism, and Participation Rubric

This evaluation assesses your professional engagement during field placement, focusing on preparedness, adherence to your Learning Contract, professionalism, and leadership. It reflects your accountability, communication, and growth as a future Addiction and Mental Health Worker or Social Service Worker.

Rating Scale:

  • Exemplary (5): Demonstrates outstanding effort and professional skill.

  • Adequate (3): Meets expectations with some room for improvement.

  • Needs Significant Improvement (0): Falls below expectations and requires immediate attention.

Criteria:

Preparedness: Evaluates your effort and preparation for field visits and meetings, including organized materials, examples of learning activities, and active participation.

  • Exemplary (5): Highly organized, detailed work; several examples; actively participates.

  • Adequate (3): Somewhat organized; includes at least one example; moderately engaged.

  • Needs Significant Improvement (0): Disorganized; unable to provide examples; does not participate.

Adherence to Learning Contract: Measures your commitment to agreed-upon goals and responsibilities.

  • Exemplary (5): Fully follows contract; signed and dated.

  • Adequate (3): Somewhat follows contract; signed and dated.

  • Needs Significant Improvement (0): Does not follow contract; missing signature or date.

Professionalism: Reflects the quality of your work, communication, confidentiality, behavior, and adherence to agency and college policies.

  • Exemplary (5): High-quality work; respectful; maintains confidentiality; excellent attendance and punctuality.

  • Adequate (3): Moderate-quality work; generally respectful; may need reminders on confidentiality; moderate attendance.

  • Needs Significant Improvement (0): Poor quality; unprofessional; confidentiality not maintained; poor attendance.

Leadership: Assesses your engagement with feedback, self-reflection, initiative, and responsibility.

  • Exemplary (5): Uses feedback effectively; highly engaged; self-reflects; accepts responsibilities and constructive criticism.

  • Adequate (3): Occasionally uses feedback; moderately engaged; sometimes self-reflects; accepts responsibilities with some prompting.

  • Needs Significant Improvement (0): Does not use feedback; disengaged; rarely self-reflects; resists responsibility.

This rubric helps guide your professional development and identifies strengths and areas for improvement throughout your placement.
