Learning Contract Appendix C Establishes placement goals, expectations, and responsibilities. Developed collaboratively with the Agency Field Supervisor. Serves as a reference for the placement experience.
Time Logs (Ongoing Tracking) Appendix D Tracks daily or weekly placement hours, activities, and supervision. Ensures compliance with Ministry-mandated hours and program requirements. Submitted to College and Agency Field Supervisors.
Midterm Evaluation Appendix H Completed midway through placement to assess progress, identify strengths and areas for improvement, and provide structured feedback from the Agency Field Supervisor. Facilitates discussion with College Field Supervisor.
Final Evaluation Appendix H Completed at the end of placement to summarize overall performance, professional growth, and readiness for future placements. Confirms achievement of learning objectives and competencies.
Capstone ‘Passion’ Project Students explore a topic within addiction and mental health that excites them, integrating knowledge and skills from their placement. Projects (e.g., workshop, research, educational resource) demonstrate critical thinking, practice skills, collaboration, and professional growth, fostering ownership of learning and contributing to the agency and community.
Meeting Preparation, Professionalism, and Participation Evaluates professional engagement, including preparing and sending agendas, facilitating discussions, attending and contributing to meetings, and participating in Midterm and Final Evaluation meetings. Reflects accountability, organization, and professional communication skills.
