Document / Assignment Reference Purpose / Description
Learning Contract Appendix B Establishes placement goals, expectations, and responsibilities. Developed collaboratively with Agency Field Supervisor. Serves as a reference for the placement experience.
Time Logs (Ongoing Tracking) Appendix D Tracks daily placement hours, activities, and supervision. Ensures compliance with Ministry-mandated hours and program requirements. Submitted to College and Agency Field Supervisors.
Midterm Evaluation Appendix G Completed midway through placement to assess progress, identify strengths and areas for improvement, and provide structured feedback from the Agency Field Supervisor. Facilitates discussion with College Field Supervisor.
Final Evaluation Appendix G Completed at the end of placement to summarize overall performance, professional growth, and readiness for future placements. Confirms achievement of learning objectives and competencies.
Meeting Preparation, Professionalism, and Participation Evaluates professional engagement, including preparing and sending agendas, facilitating discussions, attending and contributing to meetings, and participating in Midterm and Final Evaluation meetings. Reflects accountability, organization, and professional communication skills.
Professional-in-Training Portfolio Submitted at the end of placement alongside the final evaluation. Documents learning, reflections, and professional growth throughout the placement. Supports the evaluation process and demonstrates competency development.
