College Field Supervisor (CFS) Role & Responsibilities

The College Field Supervisor (CFS) oversees and supports students throughout the AMHW Field Placement, ensuring their learning experience aligns with course outcomes, professional standards, and agency expectations.


Monitoring & Oversight

  • Oversee and monitor the process and content of student learning in the field placement.

  • Review course descriptions and learning outcomes for AMHW-310 (Field Placement 1) and AMHW-410 (Field Placement 2).

  • Maintain ongoing contact with both the student and Agency Field Supervisor (AFS) throughout the placement.


Student Support

  • Conduct two formal placement visits per student each semester (midterm and end-of-semester), supplemented with other forms of contact (phone, email, or Zoom/on-site visits).

  • Document all student field placement visits using program forms and share copies with the student and AFS.

  • Assist students in achieving the best learning outcomes by offering guidance, problem-solving strategies, and support navigating challenges.

  • Respond to student emails within 48 hours.

  • If questions cannot be answered, contact the AMHW Field Placement Coordinator for clarification.


Meetings & Communication

  • Schedule two virtual placement visits with the student and AFS:

    • Midterm (Week 7/8): Review and discuss Midterm Evaluation.

    • End-of-semester (Week 13/14): Review and discuss Final Evaluation.

  • Communicate proactively with the AFS regarding student performance, and notify the Field Placement Coordinator immediately of any concerns affecting the placement.


Agency Coordination

  • Initiate contact with the agency at the start of the placement to:

    • Confirm the student’s primary supervisor.

    • Share CFS contact information for inquiries or concerns.

    • Ensure the AFS understands responsibilities, including signing bi-weekly Time Logs.

    • Verify placement dates, hours, and supervision time.

    • Schedule two field placement meetings (virtual) with the AFS and student.


Safety Oversight

  • Confirm a safety plan is in place for evening hours or isolated agency locations.

  • Ensure all work-related injuries or illnesses are immediately reported and proper accident forms are completed.


Identifying Placement Concerns

  • Identify students at risk of failure at midterm (Week 7) using the Student Success Advisor “At Risk Form”.

  • Immediately notify the Field Placement Coordinator and provide detailed documentation, including examples, dates, and strategies for success.


Learning Contract & Evaluation

  • Receive a signed Student Learning Contract.

    • Important: Missing signatures result in a 0 grade.

  • Review and approve minor changes to learning activities with agreement from all parties.

  • Monitor student progress toward learning goals and provide ongoing guidance.

  • Collect completed and signed midterm and final evaluations from the student and AFS.

  • Facilitate and coordinate all tasks related to the Final Evaluation.


Grading & Student Support

  • Assign the final grade based on student work, midterm, and final evaluations.

  • Enter the final grade into Luminate.

  • Support students with concerns or issues in the context of Reflective Practice 1 and 2 courses.

  • Participate in the Grade Appeal process if a student challenges their grade.

  • Maintain accurate records of all student and agency contacts, making records available as required.

  • Provide detailed written feedback for failing grades, including issues, examples, and dates.
