Pre-Placement NARs

CHC

Students are encouraged to begin and complete these requirements prior to the start of semester  3 to ensure they are current for the duration of  your placement. Students must complete this process through the Mohawk Pre-placement Office. Students may be required to submit the information compiled to their placement site(s) as well and are encouraged to keep the original copies in a safe place, as Mohawk will not share the information collected.

Students are responsible for the costs associated with their pre-placement requirements*:

  • a Police Information Check (approximately $35.00)
  • Immunizations
  • Flu shot, as required by individual  placement sites
  • CPR

As well there are free online MyCanvas certificates that need to be completed as part of the pre-placement requirements*:

  • Consents
  • FIRE & WHMIS
  • AODA & Workplace Harassment and Workplace Violence
  • Worker Health and Safety Awareness in 4 Steps

It is the responsibility of the student to complete the pre-placement requirements in a timely manner and be cleared with the college prior to placement courses. If a student does not fulfill the pre-placement requirements, they will not be assigned a placement site and will not be completing the placement course. Also note, that most of the pre-placement requirements (certificates) have expiry dates, it is the responsibility of the student to ensure the requirements are current for the duration of their placement courses. If a requirement expires, the student may be removed from the site and/or withdrawn from the course. If a student knowingly allows a requirement or certificate to expire while on placement, this may be considered academic dishonesty and will be dealt with as such. Additionally, if a student submits a falsified document, this will be considered academic dishonesty, and will be dealt with as such.

Students must be fully registered in their clinical course prior to the start of placement.

*More detailed information will be given to students at their Semester 3  orientation.

CVT

MASK FIT TESTING REQUIREMENTS

Mask fit testing and training is a requirement for all CVT students and is valid for two years from the date of issue. The masks are for your protection when working with patients in certain types of isolation or during a pandemic event. All students are required to have a valid certificate for the duration of their academic program.

Students who are not incompliance with this requirement, may be denied or removed from their clinical placement.

Mask fit testing will be done through Pre-Placement Services at Mohawk College’s IAHS Campus. Mask fit testing must be completed prior to attending clinical. All expenses related to mask fit testing must be absorbed by the student.

You cannot eat, drink, smoke or chew gum for 15 minutes prior to the test. Students are required to be clean-shaven at the time of the test.

Mask fit testing is required every 2 years or sooner if medical conditions or facial structure change significantly so to adversely affect the seal and fit of the respirator or CSA standard Z9404-02. Such conditions include:

  • Significant weight gain or loss (includes pregnancy weight gain) i.e. greater than 10%
  • Dental changes such as dentures or major reconstruction
  • Facial scarring
  • Facial cosmetic surgery.

Students are responsible for reporting any of the above conditions to Health Services and undergo re-testing to ensure a proper respirator fit. Students with other medical conditions that may interfere with the application and use of a respirator are responsible for notifying Health Services.

Students will be required to complete an online MyCanvas module and questionnaire, and must print and present the certificate to The Square in order to register for their course. The MyCanvas module can be accessed via Open Offering -> Mask Fit.

CARDIOPULMONARY RESUSCITATION (C.P.R.) / AED CERTIFICATION and STANDARD FIRST AID CERTIFICATION

All students, including advanced standing, are required to have current C.P.R. Heart and Stroke certification at the BLS level/AED (Basic Life Support level) prior to semester 2 of the Cardiovascular Technology program.

In accordance with the Heart and Stroke Foundation of Canada guidelines, annual recertification is the expected standard.

Students must be able to provide documentation of current certification prior to entering HSCI CVT36 – Exercise Testing and Clinical Practice 1.

All students are expected to maintain their C.P.R. certification at the BLS level throughout the program. Please note: annual re-certification is required.

Students are also required to hold valid Standard First Aid Certification.

MEDICAL CLEARANCE

The Ontario Public Hospitals Act requires all students working in a hospital setting to meet certain criteria related to surveillance for infectious diseases. Detailed medical information, including a record of completion of required immunizations, will be required upon acceptance as a student in the CVT Program.

The medical requirements form is available online through MyMohawk -> Academics -> Pre-Placement Services.

Some medical requirements (TB testing) must be updated annually, please refer to your Immunizations form for details and ensure all parts are completed in full.

POLICE CLEARANCE

All students are required to have a Vulnerable Sector Police Records check (VSS) completed annually.

Letters of request are available outside the Pre-Placement Office at the IAHS campus, which the student then takes to the police station with their application.

MYCANVAS MODULES

All students are required to complete the following MyCanvas modules (available via Self Registration) and submit the printed certificates to Pre-Placement Services:

  • Consents
  • FIRE & WHMIS
  • AODA & Workplace Harassment and Workplace Violence
  • Worker Health and Safety Awareness in 4 Steps

INTERPROFESSIONAL EVENT (IPE)

The School of Health hosts an annual Interprofessional Event (IPE) for all programs each Fall semester. It is a mandatory event for all year 2 CVT students, and is a pre-clinical requirement for CLIN 10031. Event and registration details will be made available in the Fall.

COMPTRACKER

Clinical Practice courses utilize CompTracker (www.studentlogbook.com), this application assists tracking students’ progress in completion of clinical competencies as well as providing feedback on performance.

Students are responsible for a one-time registration fee prior to the start of Clinical Practice 1.

Further details will be provided during clinical orientation.

SUMMARY OF PRE-PLACEMENT NARS AND DUE DATES

Students must have all of their pre-placement non-academic requirements (both medical and non-medical) validated by Pre-Placement Services prior to the start of their clinical placements. All details, instructions and forms are available on MyMohawk. Assistance is also available through the Pre-Placement Office at the IAHS campus.

All requirements for CLIN 10055 and CLIN 10031 are due by the deadlines indicated on MyCanvas.

Note: Any student returning after any type of leave (withdrawal, advanced standing, leave of absence, etc.) must re-start the pre-clinical process.

Special notes:

  • Clearances are the responsibility of the student. Students must be fully cleared before attending clinical. All clearances must be valid for the duration of the clinical placement. Students who are not in compliance will be denied and/or removed from clinical placement. If a student knowingly allows a requirement or certificate to expire while on placement, this may be considered academic dishonesty and will be dealt with as such. Mohawk College reserves the right to do so at the discretion of the Associate Dean of the program in which the student is registered. Additionally, if a student submits a falsified document, this will be considered academic dishonesty, and will be dealt with as such.
  • Proof of current CPR Level HCP certification must be provided to the professor of HSCI CVT36 Exercise Testing during the first week of Semester 2. If a student fails to provide this, they will not be permitted to participate in the labs, and will forfeit any corresponding marks.
  • Students must be fully registered in their clinical course prior to the start of placement.
  • Students must be registered with CompTracker prior to the start of Clinical Practice I.

MT

You must complete these requirements as a component of your Intro to Clinic course in semester 2 (prior to beginning semester 3 student clinic):

Fall Intake – Submit by beginning of April in semester 2

Winter Intake – Submit by beginning of August in semester 2

 

Many of these preplacement requirements must be updated annually in order to continue to participate in the clinics and outreaches. You are responsible for ensuring you are up to date as many of these certificates expire prior to entering semester 6. If a student submits a falsified document, this will be considered academic dishonesty, and will be dealt with as such.

OTA/PTA

Students are required to begin and complete these pre-placement non-academic requirements (NARs) prior to their upcoming placement date to ensure currency for the duration of their placements. Students must complete this process through the Mohawk College Pre-Placement Office. Students may be required to submit the information compiled to their placement site(s) as well and are encouraged to keep the original copies in a safe place, as Mohawk will not share the information collected.

Students are responsible for the costs associated with their pre-placement requirements*:

  • a police clearance check (Vulnerable Sector) (approximately $35.00)
  • mask fit test (approximately $25.00)
    • Students will be required to complete an online MyCanvas module and questionnaire, and must print and present the certificate to The Square in order to register for their course. The MyCanvas module can be accessed via Open Offering -> Mask Fit.
    • Students are required to be clean-shaven at the time of the test.
  • First Aid/CPR-BLS certificates (various costs depending on where offered)
  • Immunizations
  • MyCanvas modules:
    • Consents
    • FIRE & WHMIS
    • AODA & Workplace Harassment and Workplace Violence
    • Worker Health and Safety Awareness in 4 Steps
  • Interprofessional Event
  • CompTracker registration

Remember, you will NOT proceed to placement if evidence of these documents is not in your Nirv profile by the specified deadline. Placements may not be available outside of the scheduled semester dates, and students not able to proceed to placement may be required to sit out for the year until the placement course is offered again.

Students must be fully registered in their clinical course prior to the start of placement. Students must be registered with CompTracker prior to the start of each clinical placement.

Students will be notified of their upcoming clinical placements in advance of the start date. You will be provided with your preceptor’s name and contact information and are responsible for contacting your preceptor at least 5 working days before the start of the placement. This will enable the preceptor to communicate any specific information around start time, location of the placement and any additional pre-placement expectations of students. Students should seek advice from the Clinical Fieldwork Coordinator if they are having difficulty getting in touch with their preceptors.

Some facilities organize a group orientation for all students placed at their facility. This orientation may occur before the placement starts. Students are required to attend this orientation.

 

PHARM TECH

Students are encouraged to begin and complete these requirements while in their second semester (end of February) to ensure current for the duration of the second year placements. Students must complete this process through the Mohawk Pre-placement Office. Students may be required to submit the information compiled to their placement site(s) as well and are encouraged to keep the original copies in a safe place, as Mohawk will not share the information collected.

Students are responsible for the costs associated with their pre-placement requirements*:

  • a Police Information Check (approximately $35.00) (some sites may specifically request Vulnerable Sector Screening – VSS)
  • Mask Fit test (approximately $25.00)
    • Students will be required to complete an online MyCanvas module and questionnaire, and must print and present the certificate to The Square in order to register for their course. The MyCanvas module can be accessed via Open Offering -> Mask Fit.
    • Students are required to be clean-shaven at the time of the test (as appropriate).
  • First Aid/CPR-BLS certificates (various costs depending on where offered)
  • Immunizations
  • Flu shot, as required by individual clinical sites

As well there are free online MyCanvas certificates and face-to-face activities that need to be completed as part of the pre-placement requirements*:

  • Consents
  • FIRE & WHMIS
  • AODA & Workplace Harassment and Workplace Violence
  • Worker Health and Safety Awareness in 4 Steps
  • Patient Safety Math Test
  • Interprofessional Event/Course
  • COVID-19 Assessment and PPE

Clinical sites may have their own additional requirements (resumes, orientation training sessions, etc.).

The “Patient Safety Math Test” is a pharmacy math test that needs to be completed with 100% accuracy prior to being considered for placement. Details and process will be communicated to students in the first semester. If unsuccessful with the given 3 attempts of the math test, the student will take a remediation lesson where 2 more attempts will be planned. If unsuccessful after the 5th attempt, students will not be able to go onto placement courses (PM991/PM993), these students will be encouraged to retake the pharmacy calculations courses (PM102/PM202) and will have to sit out for a semester until placement courses are offered again.
