Section 5: Evaluation, Grading, and Promotion

Missed Evaluations

(The “Missed Evaluation Form – Allied Health” is available in the Appendix.)

If an evaluation (quiz/test/exam/presentation) is missed, the student will receive a mark of “0” for that evaluation. A student who misses an evaluation due to extenuating circumstances, may submit details with evidence using the Missed Evaluation Form (linked above) within 2 days of returning to class, to the respective course faculty member, which will be reviewed in consultation with the Program Coordinator. Only truly extenuating circumstances will be considered. The student will receive a reply from the course faculty member. This decision concerning missed evaluations will be final.

If a student has a conflict between a scheduled evaluation and a college varsity or other elite level athletic activity, it is the student’s responsibility to submit the Missed Evaluation Form (linked above) to the respective course faculty member, as early as possible, but no later than 2 weeks prior to the scheduled evaluation. The course faculty member will review the request in consultation with the Program Coordinator, with potential consideration to have the test prorated (where possible), or for the student to write the test on an alternative date and time. Failure to follow the required procedure will result in a mark of “0” for that evaluation.

Any absenteeism from the final examination will be dealt with on an individual basis, following the same procedure as above.

Procedure:

  1. The student completes Page 1 of the Missed Evaluation Form and submits the form (both pages) to their course faculty member.
  2. The faculty member determines if any medical or personal documentation is required.
    1. If yes, the faculty member will communicate this to the student
    2. The student will submit their required documentation to the Student Wellness Documentation Portal
    3. Please note, students should never send medical or personal documentation directly to their teacher.
    4. The Health and Wellness Centre team will review and provide a verification of documentation email to the faculty/instructors you have listed (the email will confirm receipt of the documentation, verify that the documentation is legitimate, and specify the dates to which the documentation applies; no personal information will be shared)
  3. The faculty member will complete Page 2 of the Missed Evaluation Form, and return a fully signed copy to the student within 1 week of submission/confirmation of documentation.
  4. The faculty member will submit a copy of this completed form to the Allied Health office via email (alliedhealth@mohawkcollege.ca) for filing in the student administrative file.
